Cockcroft–Gault CrCl Guide (µmol/L inputs)
Formula & units
We use the Cockcroft–Gault equation with SCr entered in µmol/L. We convert to mg/dL using ×0.0113 before computing CrCl. The output is in mL/min.
Weight selection
- IBW: Male = 50 + 0.91×(cm−152); Female = 45.5 + 0.91×(cm−152)
- AdjBW: IBW + 0.4×(ABW−IBW)
- ABW: Actual body weight
Select ABW/IBW/AdjBW based on clinical context; we display the method used alongside the CrCl value.
Frail/elderly SCr floor
Optionally floor SCr at 53 µmol/L (≈0.6 mg/dL) to avoid overestimating CrCl in sarcopenic patients; we label this with a soft warning for transparency.
Why CrCl matters here
Renal function directly influences safe hold/restart windows—especially for dabigatran and for neuraxial safety thresholds. Our tool applies conservative escalation when renal function is reduced.
